Brochure and Application Form

Formal brochures w/ application form for Design Discovery will be mailed out by the end of March. If you have called or emailed and left your address, you will get a brochure; as well, high school counselors from around the state and region will also receive brochures.

Scholarship Application,
Web Consent Form, and
Parental/Medical Care Release,
and HIPAA Release Form.

Please download and fill out the forms and postal mail to the School of Architecture as directed by the application.
